Monday, July 6, 2009 | All Day - Recurring Daily

Step 1: Submit Disability Documentation

Before service or accommodations will be issued, students must submit ADA approved documentation of a disability to SDS prior to registration/intake.  Refer to the link for "Documentation Guidelines" on this website. Documentation may be faxed, mailed or delivered in person to:

Step 2: Schedule an Intake

It is the responsibility of the student to contact SDS to schedule an intake appointment Monday-Friday between the hours of 9am and 5pm.  The office number is 313-577-1851.  In Fall and Winter semesters, SDS is open until 7pm on Monday's and Thursday's.  Appointments are scheduled after the staff has reviewed the documentation.  Students must complete an intake form which can be downloaded from this SDS website under the link to "Forms." Accommodations will be made when the student meets face to face with a disability specialist at which time the specialist will explain the policies and procedures for receiving services and accommodations.

Completing Intakes During Summer

New students for fall semester may meet with a disability specialist during summer in advance, of orientation.  This is to benefit students who may need the use of their accommodations for placement examinations.

Services for Deaf and Hard of Hearing Students

Students who are requesting interpreting services must notify SDS at least four weeks prior to the start of their first semester.

Requesting Materials in Alternative Format

Students who are requesting alternative print materials must notify SDS at least four weeks prior to the start of the semester.  Consult the "Student Handbook" link on our website to locate the link to "Alternative Media Policy and Procedures."
