DESCRIPTION:Step 1: Submit Disability Documentation
Before service or acco
 mmodations will be issued, students must submit ADA approved documentation
  of a disability to&nbsp;SDS prior to registration/intake.&nbsp; Refer to 
 the link for "Documentation Guidelines" on this website. Documentation may
  be faxed, mailed or delivered in person to:

Step 2: Schedule an Intake
It is the responsibility of the stu
 dent to contact&nbsp;SDS to schedule an intake appointment Monday-Friday b
 etween the hours of 9am and 5pm.&nbsp; The office number is 313-577-1851.&
 nbsp; In Fall and Winter semesters,&nbsp;SDS is open until 7pm on Monday's
  and Thursday's.&nbsp; Appointments are scheduled after the staff has revi
 ewed the documentation.&nbsp; Students must complete an intake form which 
 can be downloaded from this&nbsp;SDS website under the link to "Forms." Ac
 commodations will be made when the student meets face to face with a disab
 ility specialist at which time the specialist will explain the policies an
 d procedures for receiving services and accommodations.&nbsp;
Completing 
 Intakes During Summer
New students for fall semester may meet with a disa
 bility specialist during summer in advance, of orientation.&nbsp; This is 
 to benefit students who may need the use of their accommodations for place
 ment examinations.&nbsp;
Services for Deaf and Hard of Hearing Students

 Students who are requesting interpreting services must notify SDS at least
  four weeks prior to the start of their first semester.
