Person Centered Planning Training, for Individual with disabilities receiving HealthWest services, family members, Mental Health Staff and Community Providers

Description: This presentation will focus on the value of Person Centered Planning. It will explain how Person-Centered Planning supports an individual with disabilities who receives community mental health supports. The presentation will describe the purpose, values, and process of Person Centered Planning and share tools available to assist with the planning process.

Who should attend? All are welcome. This presentation will be helpful to:

  • Individuals with developmental disabilities who receive HealthWest services and supports
  • Family members of individuals with disabilities who receive services and supports
  • HealthWest staff (support coordinators, clinical and support staff)
  • Community based service providers (program managers, direct support staff, home managers, etc.)
