Covid requirements for University events:
All participants must meet all four (4) of these requirements in order to gain entry into the Ballroom.
Campus Health will not accept medical exemptions.

1. Provide valid identification
2. Masking: All participants are required to be masked throughout the event.
3. Daily screener: All participants are required to complete the Daily Screener.
4. Proof of vaccination: All participants are required to provide proof of vaccination and the booster. Proof can be in the form of a vaccination record or a photograph of your vaccination record

NOTE: You must complete the screening forms on the day of the event.
This form is only valid for the day it is completed.
